Chronic Disease Management

Chronic Disease Management

Mullumbimby Comprehensive Health Centre has a dedicated chronic disease nurse to provide management plans for patients with chronic diseases such as:

  • Asthma,
  • Coronary heart disease,
  • Chronic obstructive pulmonary disease,
  • Diabetes and
  • Renal impairment

We work with our patients on prevention of chronic diseases and provide regular checks on;

  • Cholesterol
  • Glucose and
  • Blood pressure and help patients to improve these.

We also have an excellent referral network of specialists and Allied Health professionals, both within the centre and wider afield, to help with chronic disease management.

If you have been diagnosed with a chronic disease take the time to speak with your GP about a management plan. This clinic is designed to keep you healthy and decrease the suffering from symptoms and progression of the disease.
